Medicare stops at the border
Medicare is US health coverage for care received inside the United States. In most cases it does not pay for care in another country.
People describe it simply: Medicare stops at the border. A few narrow exceptions exist near the US, but routine care in France or Italy is not covered.
What that means if you move to France or Italy
If you retire to France or Italy, day-to-day care — a doctor's visit, a prescription, a hospital stay — falls outside Medicare.
US domestic plans, including Medicare, are not structured to cover treatment while you live abroad. That is a coverage gap to plan around, not a detail to discover later.
The visa needs its own cover
There is a second reason retirees need separate cover: the visa itself requires it.
France's long-stay visa and Italy's elective residence visa both require private health insurance valid in the country for your full stay. Medicare does not meet that rule.
Keeping Medicare while you live abroad
Whether to keep paying for Medicare while you live abroad is a personal decision. It can matter if you plan to return to the US for care.
