Ambulance cost in Spain: who pays

By Covered Abroad Research Desk · Last verified July 2026

Spain's common services list, Real Decreto 1030/2006, gives patients receiving National Health System care the right to funded ambulance transport when clinical reasons stop them using ordinary transport. The bills come from elsewhere: where a third party is liable, such as an insurer after an accident, the health service claims the cost from them, and a private ambulance to a private hospital sits outside the public system.

What the public system funds

Annex VIII of Real Decreto 1030/2006 sets out the ambulance service in the national health system's common portfolio. It defines medical transport as moving patients for exclusively clinical reasons when their condition prevents them from using ordinary transport.

The right to that funded transport belongs to sick or injured people receiving care from the National Health System, in its own or contracted centres, who for physical or clinical reasons cannot use ordinary transport to reach a health centre, or to get home afterwards while those reasons persist. The annex covers transport without medical assistance on board and transport with it, by road, air or sea, using the most suitable means.

When someone else gets the bill

The same annex includes a sentence that matters after road accidents: where there is a third party obliged to pay, the health administration must claim the cost of the services from them. In practice that means a motor insurer or another liable party, rather than the patient, receiving the claim for the ambulance.

The private-ambulance trap in tourist areas

The Foreign Office's advice for Spain says to dial 112 and ask for an ambulance. It also warns that some hotels may call private doctors or ambulances to take patients to private hospitals, and that the GHIC or EHIC will not be accepted there.

The same advice notes that you are more likely to receive appropriate care in public facilities, but that some tourist areas may have none nearby. If you are able to choose, ask for the public emergency service by name.

Who is not yet in the system

The annex ties the right to funded transport to receiving National Health System care. A new arrival who is not yet registered, or who is taken to a private hospital, is outside that wording, which is where private cover does its work.

On Regency's international health plans, local ambulance to hospital is listed as full cover on all four tiers and on Essential Health. International evacuation and repatriation is an optional add-on rather than part of the base plan, and air, sea and mountain rescue is always excluded. Pre-existing conditions are excluded, and what applies is whatever your policy wording says. We are an insurance introducer, not an insurer.

Common questions

Is an ambulance free in Spain?

For patients receiving National Health System care who clinically cannot use ordinary transport, the transport is a funded common service under Real Decreto 1030/2006. A private ambulance to a private hospital is outside the public system, and the GHIC or EHIC is not accepted there.

What number do I call for an ambulance in Spain?

112. The Foreign Office's advice for Spain says to dial 112 and ask for an ambulance. If a hotel offers to call one for you, ask whether it is the public emergency service or a private provider.

Who pays for an ambulance after a car accident in Spain?

Real Decreto 1030/2006 says that where a third party is obliged to pay, the health administration must claim the cost of the services from them. That is typically a motor insurer rather than the injured person.

Does the EHIC cover a private ambulance in Spain?

No. The Foreign Office warns that some hotels call private doctors or ambulances that take patients to private hospitals, and that the GHIC or EHIC will not be accepted there.
