The cost of living in Melaka, against the state and the other heritage city
By Covered Abroad Research Desk · Last verified July 2026
The rule in writing
“Malaysia’s public system charges foreigners under the Ministry of Health’s published foreigner service rates (Fees Act 1951 fee orders): outpatient RM40, specialist RM120 and emergency RM100 per visit, with inpatient ward and procedure charges on top — so an uninsured long-stay resident pays the full foreigner schedule out of pocket.”

City against state, and against Penang island
DOSM's 2024 figures for the Melaka Tengah district, which contains the city:
- Mean monthly household expenditure — RM6,527
- Mean monthly household income — RM9,232
- Median monthly household income — RM7,415
- Gini coefficient — 0.373
- Poverty rate — 2.8%
For Melaka as a whole: expenditure RM6,212, mean income RM8,686, median RM6,891, Gini 0.337, poverty 3.0%.
The comparison people actually want is Melaka against Malaysia's other heritage city, George Town, whose district is Timur Laut on Penang island:
- Expenditure: Melaka Tengah RM6,527 · Timur Laut RM6,416
- Median income: Melaka Tengah RM7,415 · Timur Laut RM7,745
- Gini: Melaka Tengah 0.373 · Timur Laut 0.398
- Poverty: Melaka Tengah 2.8% · Timur Laut 0.4%
Melaka Tengah is marginally the more expensive of the two on household spending while earning slightly less. The two cities are close enough that cost is not the thing that should decide between them.

What these figures measure, and what they do not
Read the caveat before the numbers are useful. These are Malaysian households, surveyed by the national statistics office — not an expatriate budget. A foreign resident's spending diverges in four predictable places: rent in expatriate-favoured areas, international school fees, private rather than public healthcare, and imported food. It converges on everything else — transport, utilities, domestic groceries, local services.
Melaka Tengah's Gini of 0.373 is above the state's 0.337, which is the usual pattern: the city carries the range and the state average smooths it. The mean household income of RM9,232 sits 25% above the median of RM7,415, so a minority of higher incomes is pulling the average upward and the median is the better guide.
The poverty rate of 2.8% in the district against 3.0% statewide is unusually flat. In most Malaysian states the city records a markedly lower rate than its hinterland; in Melaka the two barely differ, which reflects how small and uniformly urban the state is.

Check my policyWhat Melaka costs against the rest of Malaysia
Set against the places most movers actually compare, on mean monthly household expenditure:
- Kuala Lumpur — RM8,178
- Johor Bahru — RM6,764
- Melaka Tengah — RM6,527
- Penang island (Timur Laut) — RM6,416
- Kuching — RM5,184
- Kota Kinabalu — RM4,726
- Ipoh (Kinta) — RM4,648
Melaka lands in the upper half. It is around RM1,650 a month below Kuala Lumpur and around RM1,900 a month above Ipoh. Anyone choosing Melaka for its heritage character should not also expect it to be the cheap option — on these figures it is not.
What this means for cover
The pattern across Malaysia holds in Melaka and is worth stating plainly: local costs scale with the local economy and insurance does not. A policy is priced on your age and your cover level, not your postcode. Moving from Kuala Lumpur to Melaka cuts the household benchmark by roughly RM1,650 a month and changes your premium by nothing at all.
Public treatment is charged at the Ministry of Health's foreigner rates — RM40 outpatient, RM120 specialist, RM100 emergency — before any ward or procedure charge, and private hospitals charge private rates.
Two limits worth settling before you move, because they apply wherever in Malaysia you land. The cover we arrange is worldwide — the United States is the one exception. And pre-existing conditions are excluded, which is a question to resolve before a move rather than after one.
