German health insurance, explained: the statutory lane and the private one

By Covered Abroad Research Desk · Last verified July 2026

Germany runs a two-lane health insurance system. In the statutory lane (GKV), per the federal portal gesund.bund.de, your fund covers necessary medical treatment, the eGK card is your proof of cover, co-payments apply with exemption routes, and you may freely choose and switch funds. Private insurance (PKV) is the second lane, open to groups defined by income and status rules set annually — a different product with different pricing logic.

The statutory lane, per the federal portal

The mechanics of GKV from gesund.bund.de: when a person becomes ill, the statutory health insurance fund covers the cost of necessary medical treatment; the electronic medical data card (eGK) serves as proof of statutory cover for treatment; co-payments apply to many statutory benefits, with exemption routes that can be applied for; and people with statutory insurance are entitled to freely choose and switch their health insurance provider. Contributions are income-based — the solidarity principle that defines the lane.

The private lane — and who can enter it

The portal's pages frame the comparison — the differences between statutory and private health insurance providers — without this page inventing the numbers: access to PKV is limited to groups defined by law (above an income threshold set annually, plus categories like the self-employed and civil servants), and pricing is risk-based rather than income-based. The honest guidance: the threshold figure moves yearly and belongs to the official sources — check the current Versicherungspflichtgrenze before treating PKV as an option, and treat the GKV-vs-PKV choice as one of the few genuinely hard-to-reverse decisions of a German move.

Where visa insurance fits

Neither lane is instant for a new arrival: enrolment follows employment or residence status, and the visa file usually comes first. That gap — the cover a consulate or Ausländerbehörde accepts before GKV/PKV membership starts — is exactly what our German employment-visa insurance guide and freelance-visa guide map, and arranging that bridge is the part we do daily. Once you're in a lane, the system's own rules above take over.

Common questions

What's the difference between public and private health insurance in Germany?

Statutory (GKV): income-based contributions, fund covers necessary treatment, eGK card, co-payments with exemptions, free switching between funds. Private (PKV): access restricted by income threshold and status, risk-based premiums, contract-defined benefits. The lanes are hard to swap later — especially back into GKV.

Who can choose private insurance?

Groups defined by law — employees above the annually-set income threshold, plus categories like the self-employed and civil servants. The current threshold figure is an official annual publication; check it rather than a blog's cached number.

What is the eGK?

The electronic medical data card — per gesund.bund.de, your proof of statutory cover when getting treatment, with additional functions on top.

Does my expat policy count as GKV?

No — international policies bridge the visa and arrival stage, and some residence titles accept them longer, but they are neither GKV nor substitutive PKV membership. Which German checkpoints accept what is exactly what our German visa-insurance guides cover.
