Kraamzorg: the Dutch postnatal care system, explained for newcomers

Kraamzorg is the Dutch institution of professional maternity care at home during and after the birth — you are entitled to a minimum of 24 and a maximum of 80 hours of it under the basic policy, paying an own contribution of €5.70 per hour at home in 2026. A kraamcentrum estimates your hours; in hospital the contribution disappears.

The rule in writing

“Anyone insured by operation of law under the Wlz — in practice residents of the Netherlands, and non-residents taxed on employment performed there — must take out a Dutch zorgverzekering. Residence or Dutch-taxed work is the trigger, not nationality and not the residence permit. A non-working spouse who becomes resident is caught with no Dutch income at all.”

What kraamzorg actually is

Zorginstituut Nederland's definition is compact: "Kraamzorg is de zorg tijdens en na de bevalling" — care during and after the birth. In practice it is a qualified maternity nurse in your home in the first days after delivery: monitoring the mother's recovery and the baby's health, supporting feeding, teaching the practical craft of a newborn, and quietly watching for the complications that turn dangerous when nobody notices them early.

For arrivals from the US and UK the institution itself is the surprise. There is no American equivalent at all, and no British one at this intensity — the system assumes a professional is present in your home daily through the first week, and Dutch postnatal care is built on that assumption: the midwife's follow-up and the consultatiebureau's baby checks all slot in around the kraamverzorgende's reports.

"Bent u zwanger en gaat u binnenkort bevallen? Dan hebt u recht op kraamzorg" — if you are pregnant and about to give birth, you are entitled to it. Entitled, as in: this is a right under the compulsory basic policy, not an upgrade.

The hours, the estimate, and the money

The entitlement is banded: a minimum of 24 and a maximum of 80 hours. Where you land in that band is set by an assessment — you register with a kraamcentrum during pregnancy and, in the Zorginstituut's words, it "makes an estimate of the number of hours you qualify for", revisited around the birth itself as circumstances change: feeding choice, a caesarean recovery, other children at home.

The money is the transparent kind. At home you pay an own contribution of €5.70 per hour in 2026; kraamzorg received in hospital carries no contribution. The hourly rate also does not touch the €385 eigen risico — kraamzorg's cost-sharing runs through this own contribution instead.

Deliver by choice in a kraamhotel or birth centre without a medical indication and the arithmetic changes: the same €5.70 hourly contribution plus the facility's own charges for being there, only partly reimbursable depending on your insurer. The system's pricing quietly favours its own default — home, or indicated hospital care.

How to arrange it as a newcomer, and the timing trap

The sequence Dutch families follow without thinking, made explicit for people who did not grow up with it: register with a kraamcentrum early in the pregnancy — most insurers and midwives advise doing it well before the third trimester, and the centres plan their staffing months out. Your midwife (verloskundige) is the natural referrer; if you have not yet found one, that is the first call, not the kraamcentrum.

The newcomer's timing trap is upstream: kraamzorg is a benefit of the Dutch basic policy, and arrivals often spend their first months on international cover — lawfully, while an IND decision is pending, or unlawfully, by leaning on the four-month window as if it were a grace period. A birth that lands in that window is a birth outside the system that funds kraamzorg. If a pregnancy and a move to the Netherlands share a calendar, arranging the basic policy the week you are able to is the single highest-value piece of administration available to you.

Supplementary note for completeness: some Dutch aanvullende packages and international plans add extra kraamzorg hours or cover the own contribution — a modest benefit worth checking if you already hold such cover, not a reason to buy it.
