Retiring in Malaysia, and the under-60 insurance rule
By Covered Abroad Research Desk · Last verified July 2026
The rule in writing
“MM2H lists health insurance as a required document, and MOTAC’s published guide states the acceptance test in writing: cover “must be from any Malaysian or foreign insurance provider with worldwide coverage”. A policy issued outside Malaysia is therefore accepted on the government’s own wording rather than on an insurer’s assurance. The insurance document sits at the immigration pass stage — the programme’s application checklist at the MOTAC intake stage carries no insurance line. Note an unresolved conflict at exactly age 60: the guide applies the requirement to applicants “aged 60 and below”, while the application guidelines booklet states “exempt for age 60 and above”. Treat under-60 as settled and the exact-60 case as open.”

The rule in writing
“Malaysia’s public system charges foreigners under the Ministry of Health’s published foreigner service rates (Fees Act 1951 fee orders): outpatient RM40, specialist RM120 and emergency RM100 per visit, with inpatient ward and procedure charges on top — so an uninsured long-stay resident pays the full foreigner schedule out of pocket.”

MM2H is the route, and it is not retirement-only
Malaysia My Second Home is what most people mean by retiring to Malaysia, though the name misleads twice over. It is not a second-home scheme in the casual sense — there is a real stay obligation — and it is not restricted to retirees. The age floor is 25.
What filters applicants is money: a fixed deposit at USD 1,000,000, 500,000 or 150,000 depending on tier, property from RM600,000 at Silver, a participation fee, and a cumulative 90 days a year in the country.

The insurance rule inverts the usual pattern
Everywhere else, getting older means more conditions. MM2H does the opposite.
Health insurance is a listed document for applicants below 60 years old. The official guidelines state the exemption in terms: exempt for age 60 and above.
Two details most write-ups get wrong. The document sits at the immigration pass stage, not on MOTAC's intake checklist — so reading the intake list alone will not show it. And renewal asks for it again, alongside a current medical report.

60-second check
Not sure the policy you have meets your destination's written rule? Run it through the policy check now, before you build the rest of the file around it.
Check my policyWhy the exemption is not good news
If you are 60 or over you can satisfy MM2H with no policy at all. It does not follow that you should.
Malaysia bills non-citizens on the Ministry of Health's published schedule under the Fees Act 1951 — RM40 outpatient, RM120 specialist, RM100 emergency per visit, with ward and procedure charges on top. Private hospitals set their own, higher rates and routinely ask self-paying patients for a deposit before admission.
The exemption removes a requirement from a form. It does not remove a hospital's deposit request, and it does not change who pays.

Under 60: buy cover you intend to keep
If you are below the threshold, the practical consequence of renewal re-checking is that a one-year certificate bought to clear the file is a problem deferred rather than solved.
Make sure it is residence cover — a travel policy bought for a long trip may clear a first check and will not survive scrutiny at renewal. MOTAC’s published guide states the test in writing: cover “must be from any Malaysian or foreign insurance provider with worldwide coverage”, so a policy issued outside Malaysia is accepted on the published wording. Check the area of cover on your own certificate before it goes in: The cover we arrange is worldwide — the United States is the one exception.
What to check either way
- Regional cover, because complex cases cross to Singapore about an hour away.
- Direct settlement with the private hospitals near where you will live — that is what avoids the deposit.
- Age limits: ours are Health 70, Essential 80, Life 65. Worth checking against your timeline before you apply, not after.
- Renewal, if you are under 60 — the policy needs to still be there next year.
We are an insurance introducer, not an insurer. Pre-existing conditions are excluded and cover is governed by the Regency Assurance policy wording.
