Taxas moderadoras: mostly abolished, with one door still charging

Reading Decreto-Lei 113/2011 in its consolidated form — the frame law on SNS user fees, as amended — the current shape is clear. Primary care went free: Lei 84/2019 dispensed fees in primary-care consultations, and Decreto-Lei 96/2020 extended the dispensation to all diagnostic and therapeutic exams prescribed within primary care. Then Decreto-Lei 37/2022 (effective 1 June 2022) narrowed what remained to one place: fees apply only in hospital emergency services (urgência) — and even there the fee is waived when the SNS referred you to the urgência, or when the episode ends in admission.

The frame law also caps the amounts: fee values cannot exceed one third of the SNS price table for the service. The practical translation: an SNS user pays at the emergency door only when they walked in without a referral and were not admitted.

Article 4's exemption list survives all the amendments, and it is broad: pregnant women and new mothers, minors, users with a certified incapacity of 60% or more, and users in economic insufficiency (together with their household dependants), among further categories. For a family moving to Portugal, the practical readings are immediate: children never pay taxas, and pregnancy care carries none.

Exemption by economic insufficiency runs on declared household income under the article-6 mechanics — a status Portuguese residents request rather than receive automatically.
