The cost of living in Langkawi, against Kedah and the mainland cities

By Covered Abroad Research Desk · Last verified July 2026

Langkawi households spend a mean RM4,356 a month against RM4,028 for Kedah as a whole — an island premium of about 8%. That is modest, and it leaves Langkawi below Ipoh (RM4,648), Kota Kinabalu (RM4,726) and Kuching (RM5,184) on household spending.

The rule in writing

“Malaysia’s public system charges foreigners under the Ministry of Health’s published foreigner service rates (Fees Act 1951 fee orders): outpatient RM40, specialist RM120 and emergency RM100 per visit, with inpatient ward and procedure charges on top — so an uninsured long-stay resident pays the full foreigner schedule out of pocket.”

Island against state, and against the mainland cities

DOSM's 2024 figures for the Langkawi district:

  • Mean monthly household expenditure — RM4,356
  • Mean monthly household income — RM6,278
  • Median monthly household income — RM5,083
  • Gini coefficient — 0.335
  • Poverty rate — 6.0%

For Kedah as a whole: expenditure RM4,028, mean income RM5,793, median RM4,895, Gini 0.334, poverty 8.4%.

The island costs about 8% more than its own state to run a household, and earns slightly more: a median of RM5,083 against Kedah's RM4,895. Its poverty rate of 6.0% is well below the state's 8.4%.

Against the mainland cities, on the same measure:

  • Langkawi — RM4,356
  • Ipoh (Kinta) — RM4,648
  • Kota Kinabalu — RM4,726
  • Kuching — RM5,184
  • Kuala Lumpur — RM8,178

Why the island premium is smaller than expected

Read the caveat before the numbers are useful. These are Malaysian households resident on the island, surveyed by the national statistics office — not a visitor's budget and not an expatriate one. A resident foreigner's spending diverges in the usual four places: rent in areas favoured by foreigners, schooling, private rather than public healthcare, and imported food.

That distinction matters more in Langkawi than almost anywhere else in Malaysia, because the visible economy is a visitor economy. Resort prices are not household prices, and a figure of RM4,356 a month describes people living on the island year-round, not people holidaying on it.

Langkawi's Gini of 0.335 is almost identical to Kedah's 0.334, which is unusual — islands and tourist districts often record wider spreads than their hinterland. Here the distribution is essentially the state's.

The cost that does not appear in the household survey

The survey measures money. The thing that should shape a decision to live on Langkawi is not in it.

Langkawi is an island, reached by ferry or by air. Routine care is a local question; anything that needs a larger facility becomes a transport question first and a medical one second. That is a different planning problem from living in Ipoh or Kuala Lumpur, and it is worth settling before a move rather than during one.

Two consequences follow directly from how cover is structured:

  • Evacuation cover is an optional add-on, not standard — up to US$100,000 for transfer between facilities. If distance from a larger hospital is part of why you are weighing the island, this is the line to read.
  • Air, sea and mountain rescue are always excluded. Those are a different service from inter-facility transfer, and no version of the policy covers them.

Neither point is an argument against Langkawi. They are the two questions the household figures cannot answer.

What this means for cover

The pattern across Malaysia holds on the island and is worth stating plainly: local costs scale with the local economy and insurance does not. A policy is priced on your age and your cover level, not your postcode. Moving from Kuala Lumpur to Langkawi cuts the household benchmark by roughly RM3,820 a month and changes your premium by nothing at all.

Public treatment is charged at the Ministry of Health's foreigner rates — RM40 outpatient, RM120 specialist, RM100 emergency — before any ward or procedure charge, and private hospitals charge private rates.

Two limits apply wherever in Malaysia you land. The cover we arrange is worldwide — the United States is the one exception. And pre-existing conditions are excluded.

Common questions

What is the cost of living in Langkawi?

DOSM's 2024 district figures give mean monthly household expenditure of RM4,356 for Langkawi against RM4,028 for Kedah as a whole. Mean household income is RM6,278 and the median is RM5,083.

Is Langkawi expensive to live on?

Less than its reputation suggests. At RM4,356 a month it sits below Ipoh, Kota Kinabalu and Kuching on household expenditure, and about 8% above its own state. Resort prices are not household prices.

Is Langkawi cheaper than Kuala Lumpur?

Substantially. Mean monthly household expenditure is RM4,356 on Langkawi against RM8,178 in Kuala Lumpur, a difference of roughly RM3,820 a month.

What should I plan for medically on Langkawi?

Distance rather than price. The island is reached by ferry or air, so anything needing a larger facility becomes a transport question. Evacuation cover is an optional add-on of up to US$100,000 for transfer between facilities, and air, sea and mountain rescue are always excluded.

Does living on Langkawi make insurance cheaper?

No. International health cover is underwritten on age and cover level, not location within a country. The lower household costs are real; the premium is unchanged.
