How much is a dentist in Italy, and who pays for it?

Italy runs its health service regionally, and dentistry is the clearest example of what that means for your budget. National rules say who is entitled to dental care through the Servizio Sanitario Nazionale (SSN). Each region and each local health authority then decides how that entitlement is delivered and what it publishes as its own prices.

Regione Veneto put the consequence plainly in its 2020 decision on outpatient dental services: the dental treatments carrying an access note are not deliverable to people who fall outside the three categories identified as essential levels of care by the DPCM of 12 January 2017, so those citizens can obtain them only through private practice at the provider's own tariff (DGR n. 666 del 26 maggio 2020, Allegato B, Bollettino Ufficiale della Regione del Veneto).

So the honest answer depends on three things: whether you fall inside an entitled category, which region you live in, and whether you use a public clinic or a private practice.

The Ministry of Health decree of 9 December 2015, published in the Gazzetta Ufficiale on 20 January 2016, sets the conditions under which outpatient dental care is delivered within the SSN. It works through three groups.

• Children aged 0 to 14. Preventive programmes, monitoring, treatment of decay, and orthodontic correction in severe cases.
• Health vulnerability. Patients whose medical condition makes dental care necessary. The decree names groups such as people waiting for or recovering from a transplant, people with severe immunodeficiency, people undergoing chemotherapy or radiotherapy, and people with severe coagulation disorders.
• Social vulnerability. People whose economic situation blocks access to dental care. The decree leaves the test to the regions, which normally use an income or ISEE threshold.

For these groups the decree excludes "i manufatti protesici e degli interventi di tipo estetico", meaning the prosthetic appliances themselves and cosmetic work. Source: Ministero della Salute, Decreto 9 dicembre 2015.

Two items in the 2015 decree are not restricted to the vulnerable categories.

• A dental examination, which the decree frames as including the early detection of tumours of the oral cavity.
• Immediate treatment of dental emergencies with direct access, which the decree defines as treatment of acute infections, bleeding and acute pain.

That second line is worth remembering in your first weeks in the country. Regione Veneto's 2020 decision requires the associated dental services to be delivered within the time the patient's clinical condition needs, and in any case within 48 hours.

Where that care is delivered, at what hours and by which sites is decided locally, so ask your own azienda sanitaria rather than assuming Rome's or Veneto's arrangements apply where you live.

Private practices set their own fees and publish nothing centrally. Public health authorities do publish theirs, which makes their tables a usable reference point. ASL Roma 1 publishes a 2025 dental tariff for its own clinics; the figures below are the column it applies to people who fall outside the entitled categories.

The ASL Roma 1 table carries a note that matters more than any single price: the cost of the prosthesis, the implant and the materials is always borne by the patient, including in cases of social vulnerability, health vulnerability and for children aged 0 to 14.

That is why the denture row above separates a treatment fee from a charge for the appliance, and why implant work appears in the table as several separate line items rather than one number. There is no single price of an implant in Italy to quote, and a quote that hides the split is worth questioning.

Some regions add a contribution of their own for specific groups. Regione Veneto, for example, sets a fixed contribution towards dentures for residents aged 65 and over whose household income sits below a stated threshold, repeatable every four years. Check what your own region offers rather than assuming another region's rules travel.

Which exemption band you sit in decides whether you see the full tariff at all. ASL Roma 1 sets its bands using the ISEE, the Italian means-test indicator: full exemption from the charge below an ISEE of €8,000, a reduced band from €8,000 to €13,000, a further band from €13,000 to €20,000, and the extra-category tariff above that. Children aged 0 to 14, and people with certified health vulnerability, access the entitled tariffs whatever the household income.

The same authority states that dental care is provided free of the ticket only to citizens in a condition of social vulnerability, and that prostheses and orthodontic therapy stay outside the exemption.

Other regions apply different tests. Regione Veneto works from the previous year's gross household income and issues its own exemption codes at the district office. Ask your local health authority which test it uses and which document proves it. Source: ASL Roma 1, Ambulatori odontoiatrici.

Dental costs tend to land early for a family that has just moved, because the move interrupts the routine check-up cycle and because registration paperwork takes time.

Two practical steps help. Register with the SSN as soon as your permit and residence paperwork allow, since both entitlement and exemption run off that registration. And ask for a written treatment plan (piano di cura) with an itemised quote (preventivo) before any work starts, so you can see the treatment fee separately from the cost of a crown, denture or implant.

On the insurance side, private medical cover held for a visa or for the first year in Italy deals with medical care while the public entitlement is being arranged. Routine dentistry is treated separately from medical benefits on most private plans and is commonly limited or excluded, and accident-related dental treatment usually sits under a different heading again. Read your own benefit schedule and its limits before you rely on it; what is covered is whatever the policy wording says.

One local rule worth knowing: Regione Veneto states that a patient who fails to attend a booked appointment without giving proper notice must pay for it at the rate in the current nomenclatore tariffario, even when exempt from charges. Cancel appointments you cannot keep.
