Healthcare in Sabah: why East Malaysia is a separate question
By Covered Abroad Research Desk · Last verified July 2026
Borneo gets its own health profile, and that is the tell
The UK's travel health service does not publish one Malaysia page. It maintains Malaysia and Borneo (Malaysia) separately, which is itself a statement that the risk picture differs.
The clearest difference is malaria, and it is split by terrain rather than by state:
- Low risk — "inland areas of Sabah and in the inland, forested areas of Sarawak": awareness of risk and bite avoidance recommended.
- Very low risk — "the rest of Malaysian Borneo including the coastal areas of Sabah and Sarawak": same advice, lower again.
Neither tier makes antimalarials routine. But if your Sabah plans involve inland forest rather than a coastal resort, you are in the higher of the two brackets and that is worth raising at the pharmacy.
The eastern Sabah advisory, and what it does to cover
This is the part with financial consequences.
The FCDO "advises against all but essential travel to all islands and dive sites off the coast of eastern Sabah from Sandakan to Tawau, including Lankayan Island, due to the threat of kidnapping". It separately states that "your travel insurance could be invalidated if you travel against advice from the FCDO".
Those two sentences point at exactly the islands that draw divers to eastern Sabah. The advisory covers the attraction, not a detour from it — and invalidated means the policy may not respond at all, including for something unconnected to the reason for the warning.
Ask your insurer in writing before booking. Some offer endorsements; do not assume.

60-second check
Not sure the policy you have meets your destination's written rule? Run it through the policy check now, before you build the rest of the file around it.
Check my policyDistance is the other thing that differs
Kota Kinabalu has real hospital capacity. Much of Sabah is a long way from it.
Dive sites, forest reserves and the interior are hours from tertiary care, sometimes by boat before road. That changes what a policy needs to do: the question stops being which hospital and becomes how you get to one.
Diving compounds it. Decompression illness needs a chamber, not just a hospital, and the nearest one may not be on the same island. If diving is the point of the trip, the evacuation clause and any diving-specific exclusions are the two things to read before anything else.
Japanese encephalitis also runs year-round across Malaysia, with Sarawak the highest-risk area — relevant if your Borneo travel crosses into it.
What we can and cannot help with
To be clear about scope: we are not a travel clinic and nothing here is medical advice. We are an insurance introducer for international health and life cover for people.
One thing worth knowing from the policy side: failing to get a required vaccination is a general exclusion, so a skipped appointment can affect a later claim. Pre-existing conditions are excluded and cover is governed by the Regency Assurance policy wording.
