The rule in writing
“Indonesia’s national health scheme (JKN/BPJS Kesehatan) defines participants as every person INCLUDING foreigners who have worked in Indonesia for at least 6 months (Perpres 82/2018 Art. 1(2)). The statutory obligation attaches to working foreigners; a retiree or a remote worker paid by a foreign employer is not the case that article describes, and private cover is what serves the years the scheme does not.”

The two inputs that set your premium
International medical insurance is priced from a rate card: one axis is age, the other is plan level, and Indonesia reads from the card's rest-of-world column. Every rate is a published figure, not an underwriter's mood. Age moves the premium the way you would expect, gently through the thirties and forties and more steeply after sixty. Plan level is the bigger lever at any age: a hospital-only plan can cost a third of a fully comprehensive one for the same person, because outpatient cover, bigger annual limits and richer benefits are where the claims actually happen.
Two consequences follow. First, a quote is genuinely personal: a couple aged 41 and 44 with two children is a specific number, not a range on a brochure. Second, the honest way to reduce a premium is to choose a leaner plan level deliberately, understanding what it stops paying for, rather than hunting for a discount that does not exist on a rate card.
What never moves the price, despite what you have read
Your city does not matter: Jakarta, Canggu and a town in Sulawesi price identically, because the card knows regions, not addresses. Your visa route does not matter to the premium either; the E33G and the retirement route pay the same rates at the same ages, even though one requires insurance and the other does not. Nationality does not enter it. Neither does the hospital you intend to use, although your plan level decides how comfortably the annual limit absorbs that hospital's bills.
What can change the arithmetic is the household: children under 18 price at one flat rate each regardless of age, which routinely makes family cover cheaper than parents expect, and paying annually rather than monthly avoids the installment loading insurers add to spread payments.
Getting your number, and comparing it against the alternative
The comparison worth making is not international plan against nothing; it is international plan against paying privately, because private billing is what actually happens to uninsured foreigners here. A single serious admission in a private international hospital is an open-ended bill, and the cases Bali refers to Singapore are billed in Singapore dollars. Against that, a known annual premium is the boring option, which is rather the point.
