The rule in writing
“Malaysia’s public system charges foreigners under the Ministry of Health’s published foreigner service rates (Fees Act 1951 fee orders): outpatient RM40, specialist RM120 and emergency RM100 per visit, with inpatient ward and procedure charges on top — so an uninsured long-stay resident pays the full foreigner schedule out of pocket.”

KL's private market is genuinely deep
Kuala Lumpur is not a city where you hope to find decent care. The Malaysia Healthcare Travel Council — a body coordinating healthcare travel with accredited hospitals and government agencies — works with a network that includes well-known KL names such as Sunway Medical Centre and Pantai Hospital, alongside providers in Penang, Johor, Melaka and Sarawak.
These are hospitals accustomed to international patients: English-speaking specialists, international patient desks, and 24-hour emergency departments.

The criterion that actually matters
Most guides rank hospitals by reputation. For a resident, that is the wrong question, because on the day it counts you may not be choosing.
The useful criterion is which hospitals settle directly with your insurer. A hospital that bills your policy admits you on the policy. A hospital that does not asks a self-paying patient for a deposit before admission — a conversation that happens at the worst possible moment, often with a relative rather than you.
So the work is: pick your insurer, ask them which KL hospitals they settle with directly, and know the nearest one to home and to work before you need either.
What an ambulance does with that plan
Here is the wrinkle. The UK Foreign Office gives Malaysia's emergency number as 999 — call and ask for an ambulance. A 999 dispatch generally goes to the nearest appropriate public hospital, which is a sound clinical outcome and a decision made with no reference to your insurance.
Private hospitals run their own ambulance services. If you have cover and a preferred hospital, having that hospital's direct line saved alongside 999 is what turns your plan into the one that actually happens.

What it costs when you land in the public system anyway
Plans fail, and you may be treated publicly regardless. Malaysia bills non-citizens on the Ministry of Health's published schedule under the Fees Act 1951 fee orders: RM40 outpatient attendance, RM120 specialist, RM100 emergency, with inpatient ward and procedure charges on top.
Affordable per visit. It is the admission — the stack of room, surgeon, anaesthetist, theatre, imaging and drugs, each billing separately — that produces a number worth insuring against, in either sector.

And when KL is not enough
KL handles the overwhelming majority of what a resident needs. The exception is the top of the complexity curve — major trauma, specialist cardiac and neurosurgical work — where Singapore is roughly an hour away and is the established referral destination.
That is the moment a Malaysia-only policy stops helping. Regional cover is the sensible shape for a long-stay resident, and it is worth confirming before you choose a plan rather than discovering it during a transfer.
Our plans are international rather than Malaysian, so they follow you across that border. Pre-existing conditions are excluded and cover is governed by the Regency Assurance policy wording.
Honest limits: Cover is worldwide but excludes treatment in the United States. Pre-existing conditions are excluded, including conditions you did not know about. We disclose this before you request a quote. Consulates keep discretion, and requirements can change. We show the published rule and its source; the final decision is the consulate’s.
