Expat life in Penang, and the risks named by state
By Covered Abroad Research Desk · Last verified July 2026
The rule in writing
“Malaysia’s public system charges foreigners under the Ministry of Health’s published foreigner service rates (Fees Act 1951 fee orders): outpatient RM40, specialist RM120 and emergency RM100 per visit, with inpatient ward and procedure charges on top — so an uninsured long-stay resident pays the full foreigner schedule out of pocket.”

Rabies, named by state
Most travel advice treats rabies as a country-level yes or no. Malaysia's is listed by state, and the Foreign Office names four: Perlis, Kedah, Penang and Sarawak.
Penang being on that list is the detail worth carrying, because it is not a remote posting — it is one of the two or three places Western expats in Malaysia actually settle. The practical consequence is the ordinary one: a bite or scratch from any animal, including a domestic pet, is a same-day medical matter rather than a wait-and-see.
Current outbreak status is not something we will publish. That belongs to the Ministry of Health and to your doctor.

The haze season
The second story is seasonal and regional. The Foreign Office notes that high levels of air pollution in major towns and cities in Malaysia may make respiratory conditions worse, and that forest fires in Indonesia can cause haze between June and October.
It also notes the haze can disrupt local travel, regional air travel and schools — so it is a household planning issue, not only a health one. The official tracker is the Air Pollutant Index Management System (APIMS), run by the Malaysian Department of Environment.
As in Jakarta, the honest note: haze worsens existing respiratory conditions, and pre-existing conditions are excluded from the cover we arrange.

Check my policyCare on the island
Penang is one of Malaysia's stronger healthcare regions — the Malaysia Healthcare Travel Council, which coordinates healthcare travel with accredited hospitals and government agencies, works with providers across Penang as well as Kuala Lumpur, Johor, Melaka and Sarawak.
That accreditation network is the reason Penang attracts medical travellers, and the useful side-effect for a resident is hospitals practised at dealing with foreign insurers.

What it costs if you are uninsured
Malaysia bills non-citizens on the Ministry of Health's published schedule under the Fees Act 1951 fee orders: RM40 outpatient attendance, RM120 specialist, RM100 emergency, with inpatient ward and procedure charges on top.
A rabies post-exposure course is outpatient and repeated over days. A serious admission is the other shape entirely. Private hospitals routinely ask self-paying patients for a deposit before admitting them — which is the practical argument for a policy that settles directly.
