How the SNS works for a new resident
The SNS (Serviço Nacional de Saúde) is Portugal's national health service, and it sits at the centre of the Portugal healthcare system for expats moving here full-time. Care is delivered through public health centres (centros de saúde) and hospitals, the same network Portuguese citizens use.
Your patient identifier is the número de utente, the SNS user number. It is allocated automatically the first time you contact a public health facility, or you can request it proactively at any public health facility. It is free of charge.
A user number alone does not guarantee that your costs are covered. For costs to be covered you also need to show an identification document, a Portuguese tax number (NIF), your full address in Portugal, and a valid residence permit. Once you are registered, the SNS24 portal and app let you book appointments and manage prescriptions.

The timing gap: consulate and AIMA stages come first
New residents usually meet two checkpoints before they ever reach the SNS. The first is the consulate stage, where the visa application requires valid insurance covering urgent care and repatriation. In practice, the €30,000 Schengen standard is commonly applied here.
The second is the AIMA residence-permit appointment after arrival. At this stage, travel-grade insurance is no longer enough: applicants show full health insurance valid in Portugal, or registration with the public SNS. Applicants consistently report this second stage catching them out, because it is easy to assume the consulate-stage policy still counts.
Only once you hold a residence permit do you reach full SNS footing, the point where a número de utente plus your NIF and address can actually get your costs covered.
