The rule in writing
“Spain requires health insurance “contracted with an insurance entity authorized to operate in Spain” for the non-lucrative and digital-nomad visas — with no deductible, no copayment, no waiting period and no coverage limit. Most international policies fail this rule on registration grounds.”

Spain's rule, and why it's different
Expat health insurance in Spain has one governing rule: your policy must be contracted with an insurance entity authorized to operate in Spain, with no deductible, no copayment, no waiting period and no coverage limit. That is the strictest standard in Europe, and it applies to the non-lucrative and digital-nomad visas alike. (Source: Consulates General of Spain, official visa requirements, verified July 2026.)
Most international policies fail here, not because the coverage amount is too low, but because the insurer isn't authorized to operate in Spain, which is a registration problem, not a benefits problem. Travel insurance is expressly refused for both visas, regardless of its limit. An S1 form registered with Spanish social security counts as public cover for some applicants, which can change what private insurance still needs to fill in. We never claim a policy passes Spain's rule before your case has been checked. See how policies fail this rule in detail in the Spain visa insurance trap.
How we arrange Spain-compliant cover
We arrange cover through a Spain-compliant arrangement confirmed by our underwriting partner (confirmed July 2026). A person checks the certificate wording against the consular checklist for your consulate before you buy. If your case doesn't fit the arrangement, we say so up front.
The ladder runs from Essential through Major Medical, Standard, Comprehensive and Fully Comprehensive, one insurer across all 8 destinations, with annual limits from US$1,000,000 to US$2,000,000 depending on plan. Cover starts the same day, with no medical exam and a 14-day cooling-off period. Children aged 0 to 17 sit on one flat rate per plan. Pre-existing conditions are excluded, and treatment in the United States is not covered. New applicants are accepted to age 70, or age 80 on Essential.
Confirm the Spain-compliant arrangement on your quote call before you rely on it.
Where every answer lives
If you're applying on the non-lucrative visa, the full requirement and document list is at non-lucrative visa insurance. On the digital-nomad visa, see digital-nomad visa insurance. US applicants can start at health insurance for Americans in Spain, and UK applicants at health insurance for UK citizens in Spain.
Real prices sit at Spain health insurance cost, and how the plans interact with Spain's public system is explained at Spain's healthcare system. Run your own numbers with the calculator, check what your consulate requires with the checklist, or run your current policy through the policy check. When you're ready, book a quote call and we'll confirm the arrangement fits before you buy.
Honest limits: Cover is worldwide but excludes treatment in the United States. Pre-existing conditions are excluded, including conditions you did not know about. We disclose this before you request a quote. Consulates keep discretion, and requirements can change. We show the published rule and its source; the final decision is the consulate’s.
