The rule in writing
“Indonesia’s national health scheme (JKN/BPJS Kesehatan) defines participants as every person INCLUDING foreigners who have worked in Indonesia for at least 6 months (Perpres 82/2018 Art. 1(2)). The statutory obligation attaches to working foreigners; a retiree or a remote worker paid by a foreign employer is not the case that article describes, and private cover is what serves the years the scheme does not.”

Two systems, and which one you are actually in
Indonesia's healthcare divides more sharply than most countries a Western mover will have lived in. There is JKN, the national scheme run by BPJS Kesehatan, which covers a very large share of the population at low cost. And there is a private sector of clinics and hospitals, concentrated in Jakarta, Surabaya and the tourist areas of Bali, operating at a different price and service level.
Which one applies to you is not a preference. It is decided by how you got here.
What the regulation says about foreigners
Presidential Regulation 82 of 2018 is the governing text. Article 1(2) defines participants to include foreigners who have worked in Indonesia for at least six months. Article 6(1) requires every resident to take part.
Read together, the obligation attaches to working foreigners. Someone on the retirement route has not worked here. Someone on the remote worker permit is, by the design of that very visa, employed by a company established outside Indonesia. Neither is the person Article 1(2) describes.
That is worth stating carefully in both directions: the regulation does not bar them from Indonesian healthcare, and it does not enrol them either. It is simply describing someone else.

What foreign residents use in practice
The result is that most long-stay foreigners run on private care and pay for it directly or through insurance. In Jakarta and Bali that buys short waits, English-speaking staff and international-standard facilities. Day to day, it works well and costs far less than the equivalent in Europe or the United States.
The gap opens at the top end. Complex surgery, major trauma, specialist oncology and intensive care are where Indonesian private provision thins out, and where the referral pathway points offshore.
The Singapore pathway, and the number attached to it
For serious cases the realistic destination is Singapore — close, excellent, and expensive. This is the part of the system that catches people who priced their risk from the cost of a local consultation.
The UK Foreign Office states it without hedging in its Indonesia advice: medical evacuation can cost tens of thousands of pounds. That is the figure that should drive the decision, not the price of a GP visit in Canggu.
Its advice also gives the number to call in an emergency: 118, and ask for an ambulance.

What this means for the cover you choose
If the public scheme is not built around you and the top of the private market prices in dollars, the specification writes itself:
- Outpatient as well as inpatient — outpatient is what you will actually use
- Evacuation and repatriation, sized for a Singapore transfer rather than a taxi
- Residence cover, not travel cover — the distinction that decides most declined claims
- Direct settlement where possible, so a private hospital admits on the policy rather than on your card
Our plans are international rather than Indonesian, so they follow you across the region including that Singapore referral. Pre-existing conditions are excluded and cover is governed by the Regency Assurance policy wording.
Honest limits: Cover is worldwide but excludes treatment in the United States. Pre-existing conditions are excluded, including conditions you did not know about. We disclose this before you request a quote. Consulates keep discretion, and requirements can change. We show the published rule and its source; the final decision is the consulate’s.
