The rule in writing
“Indonesia’s national health scheme (JKN/BPJS Kesehatan) defines participants as every person INCLUDING foreigners who have worked in Indonesia for at least 6 months (Perpres 82/2018 Art. 1(2)). The statutory obligation attaches to working foreigners; a retiree or a remote worker paid by a foreign employer is not the case that article describes, and private cover is what serves the years the scheme does not.”

Why evacuation is Bali's defining medical risk
Day-to-day healthcare in Bali is better than its reputation and cheaper than home. Clinics in Seminyak, Canggu and Sanur handle the ordinary competently, and an international-standard private hospital will manage most of what a resident encounters.
The problem is not the floor, it is the ceiling. For major trauma, complex cardiac work, neurosurgery and specialist oncology, the island's provision runs out — and when it does, the pathway leads off the island rather than to a bigger hospital on it.
The number the Foreign Office publishes
The UK Foreign Office does not hedge in its Indonesia travel advice: medical evacuation can cost tens of thousands of pounds. Its guidance pairs that with a direct instruction to hold cover for local treatment and unexpected medical evacuation, and to treat that as particularly important if you have a health condition.
Read it as the ceiling on your personal downside. Every other health cost in Indonesia is a rounding error beside it.

What an evacuation actually involves
Evacuation is not an ambulance with a longer route. A medical transfer from Bali to Singapore typically means an air ambulance or a medical escort on a commercial flight, a receiving hospital that has agreed to accept the patient, a doctor and often a nurse travelling with them, ground transport at both ends, and the paperwork to move a patient across an international border at short notice.
Each of those is a separate cost and a separate arrangement, which is why the question of who organises it matters as much as who pays. An insurer with a 24-hour assistance line arranges the chain. A reimbursement-only policy leaves you assembling it yourself, in the worst hours of your life, and claiming later.
Who pays, if not you
Indonesia's national scheme will not. Presidential Regulation 82 of 2018 defines participants to include foreigners who have worked in Indonesia at least six months — which is not the retiree, and not the remote worker paid by a company abroad, since the E33G route requires that employer be established outside Indonesia.
So the bill is private. It is settled by an insurer, or by you.

What to check in your policy before you need it
- Is evacuation included, or an add-on? Plenty of policies price it separately.
- Is it arranged or reimbursed? A 24-hour assistance line that organises the transfer is worth more than a higher reimbursement limit.
- Does it cover repatriation home, not only transfer to Singapore?
- Is it residence cover? A travel policy held by someone who lives here is the failure mode that surfaces at exactly this moment.
Our plans are international, include an assistance line, and follow you across the region rather than stopping at the Indonesian border. Pre-existing conditions are excluded and cover is governed by the Regency Assurance policy wording.
Honest limits: Cover is worldwide but excludes treatment in the United States. Pre-existing conditions are excluded, including conditions you did not know about.
