The rule in writing
“Indonesia’s national health scheme (JKN/BPJS Kesehatan) defines participants as every person INCLUDING foreigners who have worked in Indonesia for at least 6 months (Perpres 82/2018 Art. 1(2)). The statutory obligation attaches to working foreigners; a retiree or a remote worker paid by a foreign employer is not the case that article describes, and private cover is what serves the years the scheme does not.”

The rule in writing
“The remote worker visa (E33G) lists NO insurance requirement of any kind: the official document checklist asks for a passport, bank statement, photograph, CV, itinerary, proof of US$60,000 annual income and an employment contract with a company established outside Indonesia — and nothing else. A verified absence: no page on this site may claim this visa requires insurance.”

Your employer files it, not you
E23 is not a visa you apply for. The page states it directly: you need a penjamin — a guarantor — and the guarantor is the party that submits.
The employer creates an account at evisa.imigrasi.go.id, logs in, and files the application with your documents attached. The billing code for the government fees is generated after the sponsor submits, so even payment runs through them.
What the visa then permits is broader than the name suggests. On top of working for your sponsor, the page allows you to bring family, to study through a dual-activity notification to the issuing immigration office, and to carry out investment, tourism and family visits while the permit holds.

The document list
Described as antara lain — among others — so treat it as the floor rather than the ceiling:
- An application letter
- A sponsor's statement from the party giving you the work
- A national passport valid at least six months before expiry
- A three-month bank statement showing at least US$2,000, in the name of either the applicant or the sponsor
- A recent colour photograph, a CV and an itinerary
- A work permit from the Ministry of Manpower (izin bekerja dari Kementerian Tenaga Kerja)
That last line is the one that sets E23 apart from every other route on this site. The immigration file depends on a labour-ministry approval obtained separately, so the visa is downstream of an employment decision that has already been made and documented.
Fees by length of stay
The published non-tax state revenue charges scale with the permit:
- Up to 180 days — Rp5,250,000
- Up to 1 year — Rp7,000,000
- Up to 2 years — Rp9,500,000
Payment goes through designated banks against the billing code the system produces after the sponsor files. Because the sponsor submits, these charges usually land in a negotiation about who pays what — worth settling in writing before the application starts rather than after the billing code appears.
Ninety days to use it
Two different clocks run here and they are easy to confuse.
The visa is valid for 90 days from issue. If you have not entered Indonesia within that window it lapses, and a new visa has to be applied for — the page says so explicitly. That is separate from the length of stay, which is what the permit grants once you are admitted.
On arrival the limited stay permit and the re-entry permit are issued automatically at the point an immigration officer admits you. There is no separate trip to convert the visa.
The page also restates the limits: do not overstay the permit, do not do work the permit does not cover, and do not sell goods or services outside what the role requires.
The six-month rule the visa page does not mention
No insurance document appears anywhere on the E23 list. That is the immigration position, and it is not the whole position.
Indonesia's national health scheme is defined by presidential regulation, and it brings in every person — including foreigners — who has worked in Indonesia for at least six months. On E23 you are, by definition, that person. So of all the routes covered on this site, the work visa is the one where a statutory scheme genuinely attaches to you, just not at the visa counter and not on day one.
The gap is the first six months, and the shape of what the scheme covers afterwards. It is a domestic system, built around a domestic provider network and domestic referral paths.
What private cover adds here
Three things, on this route specifically.
It covers the opening months. Six months of work is the trigger, and the period before it is uninsured unless you arrange something.
It follows you out of the country. A domestic scheme pays inside its own system. International cover is written to work across borders, which matters for a worker who travels or whose family is elsewhere.
It covers the people the scheme does not. A spouse who joins on a family visa is not working, so the six-month rule never reaches them.
What it does not do is replace the statutory obligation. Where the scheme applies, it applies — a private policy sits alongside it rather than discharging it.
Honest limits: Cover is worldwide but excludes treatment in the United States. Pre-existing conditions are excluded, including conditions you did not know about.
