The rule in writing
“Malaysia’s public system charges foreigners under the Ministry of Health’s published foreigner service rates (Fees Act 1951 fee orders): outpatient RM40, specialist RM120 and emergency RM100 per visit, with inpatient ward and procedure charges on top — so an uninsured long-stay resident pays the full foreigner schedule out of pocket.”

What the pass is for
The Long Term Social Visit Pass is the Immigration Department's long-stay facility for people whose basis for being in Malaysia is family rather than work, investment or a residence programme. The route documented in most detail is the spouse of a Malaysian citizen.
Two conditions sit at the front of the Department's own guidance and neither is negotiable:
- The application must be submitted by the applicant together with his or her Malaysian citizen spouse. This is not a form you file alone.
- The applicant must be legally married to a Malaysian citizen and already holding a Social Visit Pass under the spouse facility. The work endorsement is a step on top of an existing pass, not a standalone application.
The Department also reserves the right to request additional documents, requires all originals to be presented for verification, and states that incomplete applications will not be processed.

The work endorsement, and why it matters
This is the feature that makes the pass valuable, and it is unusual.
Holders may take up employment, run a business, or undertake professional employment without the need to convert to an Employment Pass or a Visit Pass (Temporary Employment). On most Malaysian routes the right to work and the right to stay are separate permissions obtained separately; here the second is endorsed onto the first.
The conditions attached are straightforward. The employment or business must be lawful under the laws of Malaysia, approval to work is endorsed in the applicant's passport, and holders must comply with all regulations set by the Director General of Immigration.
Compare this with the Indonesian equivalent, where a spouse joins on a family visa and the work question is handled through a separate dual-activity notification, or with DE Rantau, where a dependant spouse simply cannot work at all.
The checklist
For the permission-to-work endorsement the Department asks for:
- A copy of the Malaysian spouse's identity card (MyKad).
- A copy of the applicant's passport and current pass.
- A completed Statutory Declaration Form with photograph, affirmed before a Commissioner for Oaths.
- A copy of the marriage certificate or marriage registration certificate.
- A letter of offer from the employer on official company letterhead.
- An employment contract stamped with an RM10 revenue stamp, stating position, duration of employment, monthly salary, and carrying the signatures of both employer and employee.
- An applicant, Malaysian spouse and employer information form.
- The latest company registration form printed directly from the MyData SSM system.
Two of those trip people up. The RM10 revenue stamp is a physical stamping step, not a fee you pay online. And the company registration extract has to come from MyData SSM directly — a photocopy of the employer's own file is not the same document.
Naziran: the part nobody mentions
The Department describes a verification activity called Naziran, carried out to ensure that holders of immigration facilities comply with the prescribed conditions.
What it involves, in the Department's own description: visits to the registered address or premises, observation, interviews with relevant parties, and review of documents submitted. The findings are then used as a supporting basis in the process of consideration, approval or extension of the facility.
So the address you register is an address someone may visit, and the marriage and employment you describe are things someone may ask about in person. None of that is a reason for concern if the file is accurate. It is a reason to keep the file accurate — particularly at renewal, when Naziran findings feed the extension decision.
No insurance on the list, and what that leaves
No health insurance document appears on the Long Term Social Visit Pass checklist.
What the absence does not do is grant access. Malaysia's subsidised public rates are for citizens; a foreign spouse is charged under the Ministry of Health's published foreigner service rates — RM40 outpatient, RM120 specialist and RM100 emergency per visit, with inpatient ward and procedure charges on top.
That is the quiet asymmetry in a mixed-nationality household: two people at the same address, one inside the subsidised system and one outside it, with the same illness costing very different amounts depending on which of them has it.
Where the work endorsement is used, an employer may offer a group scheme, and that is worth checking before arranging anything privately. Where it is not used — a spouse at home with children, for instance — nothing is standing behind the bill, and that is the case worth covering first.
