The rule in writing
“Malaysia’s public system charges foreigners under the Ministry of Health’s published foreigner service rates (Fees Act 1951 fee orders): outpatient RM40, specialist RM120 and emergency RM100 per visit, with inpatient ward and procedure charges on top — so an uninsured long-stay resident pays the full foreigner schedule out of pocket.”

A pass for people already here
Every other long-stay route on this site is something you arrange before you move. RP-T is the opposite: it is a pass you qualify for by having already worked in Malaysia, and the entry condition is time served.
The published eligibility minimums are:
- Work experience in Malaysia — currently working in Malaysia under an Employment Pass for a minimum of three consecutive years.
- A valid Employment Pass with a minimum of three months' validity at the time of application or resubmission.
- Minimum monthly income — a basic monthly salary of RM15,000, excluding allowances and bonuses.
- Income tax compliance — a Malaysian income tax file number, with taxes paid for at least the most recent two years. The declared yearly basic salary must be a minimum of RM180,000, aligning with the monthly figure.
- An educational qualification requirement.
One caution is published alongside the criteria and is worth repeating: meeting them does not guarantee approval. The final decision rests with an Approving Panel.

Read the salary definition carefully
The RM15,000 figure is where applications fail on a technicality, because of two words.
It is a basic monthly salary, excluding allowances and bonuses. Expatriate packages in Malaysia are frequently built the other way round — a moderate basic plus a housing allowance, a car allowance, a schooling allowance and an annual bonus — and a package worth well over RM15,000 a month can carry a basic that is not.
The tax condition reinforces it from the other side. The declared yearly basic salary must be at least RM180,000, and that is a figure your filed returns either show or do not. It cannot be reconstructed at application time.
If RP-T is a plan rather than an immediate application, the practical implication is that the shape of your contract matters as much as its value, and it matters two tax years before you apply.
What the pass changes
An Employment Pass ties you to one employer. That is the constraint RP-T exists to lift, and it is what makes the three-year wait worth sitting out for people who intend to stay.
The pass is granted for a long horizon rather than a contract cycle, which changes several ordinary decisions — whether to buy rather than rent, whether a spouse builds a career here, whether children start a school they will finish.
It also changes the shape of the risk you are carrying. A pass tied to an employer usually comes with that employer's group medical scheme, and the cover ends with the job. A pass that is not tied to an employer can outlast the job that qualified you for it — and the cover attached to that job does not come with you.
The cover question on a ten-year horizon
Two things change when residence stops being a two-year question.
Group cover is a benefit, not a possession. It is underwritten for the employer, it ends when employment ends, and it is not portable. Someone who has spent three years inside an employer's scheme and then moves to a pass that does not require an employer has a gap to plan for rather than a policy to transfer.
Age moves against you while you wait. Underwriting is priced on age at entry, and a condition that develops while you are covered by a group scheme becomes a pre-existing condition the moment you apply for an individual policy afterwards. The three qualifying years are precisely the period in which that can happen quietly.
Underneath it, the ordinary exposure does not change. Malaysia charges foreigners at the Ministry of Health's published rates — RM40 outpatient, RM120 specialist, RM100 emergency, with ward and procedure charges on top — and a residence pass does not alter that.
How it sits against the other routes
Malaysia's long-stay map has four quite different doors, and RP-T is the only one you walk through from the inside.
- RP-T — earned by three years of employment and RM15,000 basic monthly salary. No insurance requirement published.
- PVIP — bought, at RM40,000 monthly income, a RM1,000,000 fixed deposit and RM200,000 in fees. No insurance requirement published.
- MM2H — a second-home programme whose MOTAC guide requires cover for applicants aged 60 and below, from any Malaysian or foreign provider with worldwide coverage. The cover we arrange is worldwide — the United States is the one exception.
- DE Rantau — a nomad pass requiring a health insurance policy covering Malaysia, valid throughout the pass period.
The pattern is not a sliding scale. Two of the four routes ask for insurance and two do not, and it does not track cost, duration or seniority — which is why each application has to be read on its own document list rather than by analogy with the last one.
Honest limits: Cover is worldwide but excludes treatment in the United States. Pre-existing conditions are excluded, including conditions you did not know about. We disclose this before you request a quote. Consulates keep discretion, and requirements can change. We show the published rule and its source; the final decision is the consulate’s.
