Moving to Indonesia: what happens to your healthcare

The rule in writing
“The remote worker visa (E33G) lists NO insurance requirement of any kind: the official document checklist asks for a passport, bank statement, photograph, CV, itinerary, proof of US$60,000 annual income and an employment contract with a company established outside Indonesia — and nothing else. A verified absence: no page on this site may claim this visa requires insurance.”

The rule in writing
“Indonesia’s national health scheme (JKN/BPJS Kesehatan) defines participants as every person INCLUDING foreigners who have worked in Indonesia for at least 6 months (Perpres 82/2018 Art. 1(2)). The statutory obligation attaches to working foreigners; a retiree or a remote worker paid by a foreign employer is not the case that article describes, and private cover is what serves the years the scheme does not.”

The gap, stated plainly
Moving to Indonesia has an unusual shape compared with a move to Europe. In France or Spain the visa itself forces the question — no certificate, no visa. Indonesia mostly does not ask.
The official eVisa checklist for the E33G remote worker permit lists a passport, bank statement, photograph, CV, itinerary, proof of US$60,000 annual income and an employment contract with a company established outside Indonesia. No insurance line. The second home route is the same. Only the retirement route requires cover, and it asks for three kinds: health, death and third-party liability.
So for most arrivals there is no gate, no rejection, no prompt. You simply land, and whatever you were holding at the airport is what you have.

Why the public scheme does not close it
The reasonable next assumption is that residence brings you into the national system. Presidential Regulation 82 of 2018 says otherwise, at least for the people most likely to be reading this. Article 1(2) defines participants to include foreigners who have worked in Indonesia for at least six months.
A retiree has not worked here. A remote worker is employed, by the design of their own visa, by a company established somewhere else. The regulation is not excluding them out of hostility — it is simply describing a different person.

60-second check
Not sure the policy you have meets your destination's written rule? Run it through the policy check now, before you build the rest of the file around it.

Routine private care is inexpensive, which is exactly why this risk gets underpriced. The consultation you pay for in Canggu tells you nothing about the event that matters.
That event is evacuation. For complex trauma, cardiac or neurosurgical work the pathway leads to Singapore, and the UK Foreign Office states the cost without hedging: medical evacuation can cost tens of thousands of pounds. Its advice also gives the emergency number — 118, ask for an ambulance.

A sensible order to do this in
If you are moving in the next few months:
- Work out which route you are on. It decides whether insurance is required at all, and on the retirement route it decides the shape of what you need.
- Assume the public scheme is not yours unless you will be employed in Indonesia for six months or more.
- Buy residence cover, not travel cover. This is the single most common mistake and it fails at the worst moment.
- Insist on evacuation and repatriation, arranged rather than reimbursed.
- Check outpatient is included — it is most of what you will actually claim.
Where to read the detail
Each of these has its own page with the official source and the date we read it: what each KITAS route requires, the E33G remote worker visa, how the healthcare system works, what BPJS says about foreigners, evacuation from Bali and what cover costs.
We are an insurance introducer, not an insurer. Pre-existing conditions are excluded and cover is governed by the Regency Assurance policy wording.
